Recent Achievements:

Established two-tiered Chronic and Chronic-Complex patient registry. Using Pediatric Medical Complexity Algorithm (PCMA), over 18,000 patients were identified.

Established disease specific patient registry for chronic conditions, including Asthma and Primary Recurrent Headaches.

Using Healthbridge through The Health Collaborative, implemented regional utilization alert system which provides daily alerts to member practices of ED visits, Hospital admissions, and urgent care visits for over 200,000 patients.

Assisted 24 member practices (42 sites) in becoming recognized by NCQA as a Patient-Centered Medical Home covering 217,000 lives. Also, supported 2 specialty practices in becoming recognized as Patient Centered Specialty Practices.
